If I had to place my finger on one or two items that delay reimbursement to your EMS organization, I would immediately state lack of signatures or insurance information on a patient care report (PCR). Every business survives on cash flow and part of the solution in ensuring a positive cash flow starts with crews in the field obtaining not only solid patient information but also the patient's insurance coverage and signatures.

Did you enjoy that week's vacation over Labor Day? I hope so but for some of our colleagues and friends, vacation was cut short due to this lady named Dorian who devastated the Bahamas and cities along our mainland. Dorian's wrath did injured many and sadly, a number of people lost their lives. EMS was there once again to assist and work through the terror that Dorian evoked.
While we were saddened by the loss of life, we were heartened that ambulance services from across the country went south into the storm to help where they could. Not many people run into danger but EMS, police and fire step forward and come to help those affected in every case like this.
Once the storm passes and the assisting teams return home, there is still much to consider. This episode of the EMS Board & Collar pays homage to those who are always there in disaster situations. Chuck also passes along a few tips to ensure that your organization can obtain some limited form of remuneration for the time you have committed to going, caring and lending a helping hand. While the thought of Zzzzs may equate to a good night's sleep, in the EMS industry, Zs remain part of the detailed coding now required to process your PCR for payment. Z-codes play a unique role of enabling the coder to drill down to further specify a patient's illness or injury. The other day, I had a conversation with a co-worker and throughout the conversation both he and I used multiple acronyms - all which have originated from CMS (acronym for Centers for Medicare and Medicaid Services). I thought to myself, goodness, there are a ton of healthcare acronyms out there today, aren't there?

They don't occur that often but when they do, Specialty Care Transports (SCTs) present to the provider a unique cloud of uncertainty when it comes to documenting these "Misunderstood" transports. Today, nothing happens without good, solid data to backup every decision that governments and corporations make. Even professional sports teams use analytical data on such things as how to pitch to power hitters or where to position the fielders when a specific batter comes to the plate. From an EMS perspective, with good data we can look to better structure the Medicare ambulance fee schedule to reflect the true costs of providing ambulance services. Multiple trauma patients challenge our professional senses when carrying out the delivered care in the field. We are called to use all of our talents and training to ensure a positive outcome for each patient. For many reasons, this doesn't always come easy.

I truly wish I had a nickel for every time I uttered the word "Medicare." Whether we like it or not, Medicare drives the healthcare industry. As goes Medicare, so goes the other payers and so goes healthcare in the new millennium.
From an EMS standpoint, the word association game for Medicare includes terms and phrases like under-funding, largest portion of a community's payer mix and more. As time moves forward, we will have to work with groups like the American Ambulance Association to ensure our collective EMS voice is heard by CMS. Otherwise, we could go from bad to worse in the years ahead.

Every morning on my way into work, I always try to remind myself to please Gary, engage brain before mouth on all things the day may bring. Sometimes, I am really great at this and other times, I fail miserably. When you're in a patient's presence this is even more paramount. While we may think, amidst their pain and suffering, our patient may not recall things stated to them or the responses we've made to their questions. However, they do, and we need to be cognizant of this every time we have a patient in our care.
This is especially true when they ask THAT big question enroute to the hospital... "How much is this going to cost me?" Be smart about your response and don't get caught in the middle while trying to make your patient feel better through your offered words. While the matter of questionable billing practices has not made the evening news in some time (thank goodness!), it's good to re-visit the subject matter just as a simple checkpoint to make sure we are all singing from the same sheet of music. On my way into the office this morning, traffic was backed on a busy street that usually moves well. What, an accident at this time of day? Never good for making it to the office early to get a bit of work accomplished before meetings and the chronic ringing of the phone.
As traffic moved ever-so-gingerly around the accident, I noticed the EMS crew extricating an individual from one of the involved vehicles. Four medics busting their tail along with an engine company and the police department. Organized and well-orchestrated, much work was getting accomplished to extricate the patient and keep the scene safe.
Given this, I thought I would simply extend a note of thanks as this could have easily been me or one of my family involved. I know, I know, protocol is to say thanks in May during National EMS Week. Heck with that, you deserve it everyday I would be remiss if I did not state it today after seeing the tireless work all of you were putting forth this morning. Whether you're in Anchorage or Albion, Derry or DuBois, Pittsburgh or Platte Valley, thank you.
