How do you define success?
It seems like a simple question, but it's actually a cornerstone of a happy career or business.
For many of us, finances are a primary scoreboard when it comes to our job or running our own clinic. But is this really how we choose to define success - or is it just easy?
In this 'sode we talk about the importance of defining success for ourselves and not letting goalposts shift and move so we're perpetually chasing goals that might not even be ours.
If you're working for someone else, it involves understanding both your financial needs but also the other things that make you happy - time flexibility, work environment, learning, growth, and other intangibles.
If you're running your own business, this becomes even more important! You need to make sure your business is working for you. It's far, far too easy to wake up and realize you're working for your business instead.

Should you have a consistent evaluation process or should it vary significantly depending on your patient?
We've been training a new member of our team, which always leads to some great questions. One that came up this week was: "How much should your evaluation change depending on the patient?"
In this 'sode we dive into what your evaluation should look like. What kind of framework you should have and how you can modify it to provide patient-centered care.
One major point we bring up here is freeing yourself from the constraints of the 'one-visit eval.' In reality, your evaluation is likely to take up at least two visits, and maybe more, depending on the complexity of your patient.

For many patients rectal evaluation and treatment is the "missing link" in their progress. Yet the majority of patients never even have an evaluation, and many that do never have consistent treatment. Why?
Often we're not comfortable as pelvic PTs with our rectal skills. Most courses only discuss "obvious" cases - GI, colorectal, or male patients.
But these are often a small fraction of our patients; we should be looking for our pelvic pain, post-partum, or orthopedic patients in many cases!

What does it take to have a healthy practice?
We need to talk about the difference between Revenue and Profit. One good way to think about this is to ask - would you rather have a clinic that brings in $1,000,000 per year or $100,000 per year? The answer?
It depends on your PROFIT!
Revenue does not equal profit; the sooner we recognize this and put more import on profit the healthier our businesses would be.
This is also important for employees to understand! You need to have at least a basic idea of how the business side works to have constructive conversations with your employer.
In this 'sode we go over the difference between revenue and profit, how to stay profitable as a company and how to understand the finances of your business. Whether you're an employer, a solopreneur or an employee, this 'sode is going to drop some knowledge bombs!
In this case report we go through a complex pelvic pain patient who had been getting treatment for over 10 years before coming to the PelvicSanity Out of Town Program.
Her symptoms included bladder pain, urinary urgency/frequency (30+ times per day) and burning vaginal pain.
She had been diagnosed with IC and tried all the typical medical management pieces (oral medications, nerve blocks, bladder instillations, etc.) as well as to multiple physical therapists.
The biggest missing piece? Rectal evaluation and treatment.
In over 10 years of going to multiple practitioners, no one had ever done a rectal exam! On the initial evaluation we reproduced some of her major symptoms and rectal treatment became a staple of her treatment during her out of town program.
Even though there weren't any obvious "better do rectal" signs! Check out the full 'sode for all the details and what we found!

Why does our goal seem to be to kick patients out of our practice the moment we can? Is this really what's best for patients, or is this urge coming from somewhere else?
In this 'sode, we dive into this "discharge mindset" and see where it really originates (insurance companies) and whether it's actually helping patients (it's not).
If it's so easy to teach a patient to "be their own PT" in four visits, why did we spend three years in school?
Can you imagine another profession saying the things we say?
- The lawyer, before your trial: "Just another visit or two and you'll be able to write your own opening and closing statements!"
- The mechanic: "I'll change your oil for you this time, but then I'm going to show you how to do it so you never have to come in again."
- The dentist: "Here's a self-care tool for cleaning your own teeth, I hope I never see you again"
- The hairdresser: "You know, you really could cut your own hair..."
This attitude often starts in PT school, but it originates with insurance companies. And we're duped into pretending it's for the patient's benefit!
Why are we working so hard to save insurance companies money? Our patients pay (often exorbitantly) for their insurance benefits and we try not to use them!
Your patient wants expert level care (that's why they're in your office) and has the right to decide for themselves - based on your honest clinical opinion - when to come in! It's that simple.

This is a great question that we first heard from Guy Raz (How I Built This book). Once we did, the light bulb went on!!
Scary is subjective. It's stepping into the unknown. Feeling butterflies when you push the 'publish' button. Starting a business.
Dangerous is objectively, well....dangerous. It's a decision that can cause harm that can't be undone.
Many times we feel like something is dangerous, when it's really just scary.
If we start our own business and it doesn't work out, will it ruin our life? Probably not.
This thought was liberating when starting PelvicSanity. If it didn't work out, I'd just go back and find another job working for someone else. I'd literally be in the same place I began. Starting PelvicSanity was scary, but it wasn't dangerous.
What was dangerous? Being in the same job I didn't love 10 years later. It was putting a strain on my relationships and my love of our profession. STAYING PUT was the danger, but it wasn't scary.
So what's scaring you? That scared feeling is usually the tickle that tells you it's something worth doing.

Have you ever thought about the difference between mentorship and coaching? We hadn't....until a few months ago.
This lesson applies in both the clinical and business realms.
Mentorship is wisdom. It helps you grow in confidence, competence and problem-solving. A mentor can easily be someone outside of your specialty or niche that helps you see the big picture.
Coaching is tactics. It's knowledge. It's the specific skills and resources you need. A coach needs to fully understand your specialty (field, sport, whatever it is).
In both clinical skills and business, we need both!
Clinically, we need coaches and courses (which are coaching). But we also need bigger-picture thinking to understand how to apply those skills (to use the tools in our toolbox!).
The same is true in business. We need specific skills and tactics, but without the wisdom to know how to apply them we are going to struggle.

"How long until I get better?" "I don't know if I'm making enough progress." "How much longer is this going to take?"
These are some of the toughest questions we have to answer as pelvic PTs. And if we don't have a framework for how we think about treatment, these can be really difficult to answer!
But in this 'sode we go through the 4 Phases of Treatment. This is a way for you to organize your plan of care and communicate it with patients to keep their trust and buy-in throughout the process.
This also helps if you're getting a lot of 'self-discharges', when symptoms have improved enough but you don't really feel like the patient is finished.
This framework actually comes out of the online Pelvic PT Essentials course, where we go into each of these phases in more depth. But hopefully this overview gets you thinking about what phase your patient is in and how you are communicating it with them!

Wondered how to get more buy-in from patients? Have patients who just 'don't get it' or who are 'non-compliant?'
One of our favorite frameworks comes from Gretchin Rubin, who developed the Four Tendencies. They're all about how we all respond to both internal and external expectations.
We discuss this in greater detail in the Communications module of the Pelvic PT Essentials, but how you communicate with your patients is hugely important. This framework is a good way to evaluate how you're changing your style to suit each patient. Because...it's not about us!

In this case report, the patient presented with classic IC-like symptoms. Pelvic pain, urinary urgency/frequency and painful intercourse.
In working with her over the course of several months, we resolved these issues. This is where many PTs would have automatically discharged her!

We've been on both sides: the employer and the employee. It's like a coach and a ref - they're watching the same game but seeing completely different things. It's a different perspective - and that's ok!
The first thing to really assess? "Is this person an asshole?"
Not kidding! You might be working with an asshole; in that case the rules are different.
But if you aren't, then we need to do a better job of understanding each other's perspectives and having a little grace. We won't always see eye to eye, but we can have open lines of communication and respect each other's positions!

For those of you not familiar with this buzzword, lead generators are (usually) free pieces of content you give to potential customers in return for their contact information. These can be e-books, exclusive videos or anything else you can convince someone is valuable.
They do have a place, but they are being chronically misused and misunderstood. And many pelvic PTs are driving AWAY traffic with their lead-gens and actually making it HARDER for patients to schedule!

Those in the model love it, but there's a lot of criticism in certain circles about "cash physical therapy."

All. The. Time.
Does that mean we can't help them get better?
Absolutely not.
We are not the arbiter of our patient's life choices. We've had avid bicyclists with pudendal nerve irritation who are adamant they won't stop riding. New mothers who want to get back to running before they are truly ready. Women with painful intercourse who feel they can't stop having intercourse with their partner.

Have you ever explained away a patient not getting better because they were 'non-compliant' with their home program?
We've all been there - it's tempting, but I want us to get the negative, judgmental term of 'non-compliant' completely out of our vocabulary.
If your patient isn't doing what you're asking, it's on US to take responsibility and get them better. They may not understand the importance of what you're asking. They may not understand how to do what you're asking. They may feel overwhelmed, or simply not have enough time. Or they may be relying on YOU and your sessions to get better.
I'm not saying we shouldn't impress upon our patients the importance of their home program, but we can't just dismiss them if they aren't doing exactly what we ask. We need to work with them to understand why (there's the 'why' again!) and find a way to help them reach their goals, regardless of how 'compliant' they end up being.

How do you find balance?
Between work - whether you own your own place or are employed by someone else - and home life. How to maintain a social life, exercise and take time for everything we know we need.
Sometimes it feels impossible. And that's ok!
"Work-Life Balance" is an unachievable ideal. It's something we can strive for, but not a place we can stay. And sometimes even that idea makes us miserable.

Ever have a patient who wasn't in for the 'right reasons?' (Yes, a Bachelor reference, y'all!).
Maybe a patient who cared more about the appearance of their stomach than about the function of their core or pelvic floor with a diastasis?

Think of a patient who wasn't ready to do something. To come to PT, to take a medication, to commit to their own health.
Just because they've committed to coming to physical therapy does not mean they're ready to do everything you say or make major life changes.
In Part 1 of this series, we talk about how you aren't the arbiter of your patient's WHEN.
We don't get to decide when they are ready. And we have to be ok with that.
We can still give them our honest, direct clinical advice - but we don't get to dictate their decisions. And the sooner we can accept that, the better care we can give.

Are you putting unrealistic expectations on your patient? Your patient should never feel like they're "running out of time!"
In this episode, we dive into a case report from a patient who contacted us after being treated for vaginismus at another facility. There's a lot to unpack in her letter, but it's a really interesting - and sometimes heartbreaking - patient perspective.
Being able to prognosticate or forecast for your patient is such an important skill! We devote basically an entire module of the Pelvic PT Essentials course to this ability.
If you give a patient unrealistic expectations - either because of your own mistake, because your basing the number of visits on insurance reimbursement, or any other reason - you can create the feelings of "guilt and shame" this patient experienced.
The opposite of this is creating a partnership, and we love the way this patient explains what she was - and wasn't - looking for with her pelvic PT.

Are you getting in the way of your patient's success?
Despite our best intentions, we do this ALL THE TIME.
We need to understand how our own biases, experiences, and attitudes are impacting our patients.
We got really excited about going through this - we're going to be spending the next 6 episodes talking about different ways we get in our own way, and how to break that cycle.
We need to give our honest, direct clinical advice to the patient. If they can't follow those recommendations - for any of the different reasons we dive into - then we absolutely can tailor the plan to them. But don't let your assumptions and biases dictate the care your patient receives!

Controversy Alert! How do we make the maximum difference for our patients without creating dependency in patients?
One of the biggest issues we see in the field is the tendency to discharge patients too early. Their symptoms may be reduced, or their insurance benefits may run out, but the underlying 'why' hasn't been resolved. Eventually - whether it's a week or a month later - something else will happen and their symptoms will return. And then they either need to come back or they don't....finding someone else to help, having a surgery, or just feeling like they need to live with their symptoms.
We dive into the question of how to create resiliency in patients without making them feel dependent on pelvic PT. We need to see a future for patients they can't yet see for themselves, and help them meet goals they didn't even think they'd be able to reach.

Money is an uncomfortable topic for many of us. In fact, we may even dread the money conversations with our employers or employees, our significant others and family members, and even splitting the check at dinner.
But in order to really understand our profession, we need to understand the financial foundation of our system. It affects all of us, whether you're a business owner, a future business owner, or an employee. At heart, this is about how we provide value to patients in return for their money - no matter how complicated we might make it.
In this episode we dive into the different types of practices: insurance-based, out-of-network, and completely out-of-network (or 'cash') practices. Each have their pros and cons for both us as physical therapists and our patients. And in a follow-up episode we'll reveal our own thoughts on the business side and what we decided to do for PelvicSanity.

In Part 1 of this case report we discussed the telehealth evaluation of a patient presenting with a vaginismus diagnosis.
A few weeks later, we had the opportunity to actually work hands-on with her after the Coronavirus restrictions were lifted.
Getting to work with her hands-on after doing a remote consultation was eye-opening. It really showed the limitations and challenges of telehealth, and that if we're going to continue incorporating remote visits as a regular part of treatment going forward we're going to have to drastically improve our online process.
Hear about what we picked up in person that we couldn't get over a video call, how the initial hypothesis completely changed and why we didn't feel like she really even had vaginismus at all!

How do we actually do telehealth? We've spent a lot of time talking about converting patients to telehealth, but we've heard a lot less about how to provide as much value as possible for patients remotely.
In this two-part series, we examine the interesting case of a patient who had a vaginismus diagnosis and several other symptoms. Her initial consultation was done via telehealth during the Coronavirus shutdown, and then we were able to evaluate her in-person once we re-opened the clinic.
In this first part, we go through her initial evaluation. We discuss how to find clues in what patients say, what to pick up on remotely and 'how to let your patient drive, but you steer.'

We've all had those patients who see significant improvement early on, but then plateau and aren't reaching their full goals. While there can be many reasons for this, one of them can just be the way that you're communicating with them. You can be doing everything right physically, but if your patient doesn't understand the process they can be feeling frustrated and checked-out.
In this episode, we dive into a fascinating case report from a remote consultation with an IC patient who had been seen at PelvicSanity. Usually, we can only guess at what the physical therapist was thinking based on what they tried - in this case, we had her notes on hand because one of our team had been working with her!
Our staff PT hadn't missed anything in her initial evaluation or plan of care, and had gotten the patient substantially better. But because the patient didn't understand exactly what and why they were working in certain areas, she had lost faith in the process when she stopped seeing progress at every session.
It's a great reminder that you have to be constantly communicating with the patient, helping them understand the process and getting their buy-in - even if you're doing all the right things!

There's a lot of uncertainty about how to best serve our patients in this time. We need a thoughtful process to balance our responsibilities to our patients, our staff and our community.
